Document Storage
AFSC encourages members to use our vault document storage service. Having the proper documents stored at AFSC streamlines the claims process for the family if the member or another family member dies. However, we can accept only items specifically supporting or directly related to the claims process or those items that we need to provide members and their families with the best possible continuing service. Documents stored with us are only "a phone call away" and will be sent by mail, usually the same day.
What should be stored in the vault?
We encourage members to store original or certified copies of the following documents for family members: (Certified copies bear the official seal of office of original issue. Notarized copies are not "certified" copies for claims purposes.)
- Marriage record of member and spouse
- Birth record of all dependent family members
- Adoption papers
- Divorce decrees from previous marriages
- Death certificates of former spouses
- DD Form 214 (Copy # 4-Member Copy) after separation or retirement from service
To obtain an original birth, death, marriage or divorce certificate
To obtain another copy of your DD-214

- Life insurance policies (copies of the 1st two pages of the policy)
- Copies of first page and signature page of trust agreements if applicable
- Letter of Instruction to survivors if desired
